Your privacy is important. Federal and state laws require healthcare providers to protect your health information.

  • Conduct healthcare operations

  • Comply with legal requirements

You have the right to:

  • Inspect and obtain copies of your records

  • Request corrections to your records

  • Request restrictions on certain disclosures

  • Request confidential communications

  • Receive an accounting of certain disclosures

  • Receive a paper copy of this notice

I will not disclose your protected health information without your written authorization except as permitted or required by law.

If you believe your privacy rights have been violated, you may file a complaint with the Practice or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
